Voluntary Participation: I understand that Pilates involves physical movement and exercise. I am voluntarily participating and take full responsibility for my own health and safety. 2. Assumption of Risk: I acknowledge that physical activities may involve risks of injury, including but not limited to muscle strain, soreness, falls, or other physical injuries. I understand these risks and choose to participate anyway. 3. Health Disclosure: I confirm that I am physically able to participate in Pilates.
