Milagros is a JASA Care Transitions client who is originally from Puerto Rico and moved to America at the age of 14. She met her husband here and they have been married for 54 years. Recently, Milagros was hospitalized due to an infection. With the assistance of JASA, Milagros made a safe transition back home after her hospital stay. JASA’s Care Transitions specialist conducted a home visit to assess her needs. The specialist observed that Milagros lacked certain essential items and was struggling to manage the medications prescribed by her doctor. To address these issues and prevent her from being readmitted to the hospital, the JASA team devised a plan.
