The pest control company performing the pre-treatment gives this form to the builder to complete when prevention of subterranean termite infestation is specified by the builder or required by the lender, the architect, FHA or VA. This guarantee is issued by the builder to the buyer.  Form HUD-NPMA-99-A is used in conjunction with HUD-NPMA-99B (when applicable)
